How to document and escalate unsafe conditions
Build a factual record, use the correct workplace channel and make a specific safety request while protecting patient privacy.
Before you start
- Locate your employer's hazard, incident and clinical escalation policies.
- Identify your supervisor, occupational health and safety contact, worker representative and union contact if applicable.
Step by step

Stabilize immediate risk
Use the clinical or workplace emergency chain of command when harm may be imminent. Do not delay an urgent escalation to build a perfect written record.

Record observable facts
Capture date, time, location, hazard, who was notified, the response and whether risk remains. Describe workload and impact with concrete facts. Keep required patient information only in approved clinical or incident systems.

Use parallel reporting channels
A clinical concern, occupational hazard, staffing issue and professional-practice concern may use different systems. Ask where each report belongs and retain permitted confirmation numbers or copies.

Escalate with a clear request
State the unresolved risk, previous steps and the action or decision needed by a specific time. If unresolved, consult your joint health and safety representative, union or professional association, practice leader and the applicable government safety authority.
